2. Charter (New Club) Payments – summary sheet for all Membership Applications (Form 3) – updated 9/2024
3. Membership Application (pre-fill as much as possible before distributing to potential members) – updated 2/2023
4. Officer Information (submit as soon as officers elected) – updated 4/2020
5. Club Information (submit as soon as club name and day/time/frequency of meetings are determined)
6. Club Constitution 6A  & Constitution 6B Addendum of Standard Club Options –  updated  10/2022
